CLEAR EXPECTATIONS

Your information matters.

Prototype — real intake is disabled.

The final privacy notice, institutional approval, and retention schedule must be established before real requests are accepted.

What the request form asks for

Your library card number, first and last name, telephone number, and a description of the problem. You do not need an account or email address.

How the information is used

To review your request, contact you by telephone, and maintain a private assistance history. Staff may add internal notes and follow-up dates. A card number helps organize requests; it does not verify identity or provide access to library systems.

Who can access records

Only specifically authorized staff may access patron records through the protected dashboard. There is no public patron search or online request tracking. The planned infrastructure uses Cloudflare Workers, D1, Access, and Turnstile. Cloudflare processes technical connection and security information to operate these services.

What to leave out

Do not submit passwords, payment information, Social Security numbers, medical details, or sensitive account information. This site does not request file uploads.

Retention and corrections

An approved retention period and a process for requesting corrections or removal will be published before real intake opens. The prototype includes staff tools for correcting and deleting records; production policies must also address backups and provider retention.

About the service

This independently operated website is intended for library patrons. It is not an official Colonial Heights Public Library website. Submitting a request does not guarantee a response time, appointment, or resolution. For urgent matters, contact the appropriate service directly.
